Clarification Text Regarding Personal Data Processed in the Contact Form

This clarification text has been prepared by Levent Hospital A.Ş. as the data controller, in accordance with Article 10 of the Law on the Protection of Personal Data No. 6698 (“KVKK”) and the Communiqué on the Procedures and Principles for Fulfilling the Obligation of Disclosure. It aims to inform you about the purposes of processing your personal data, legal grounds, collection methods, who it may be shared with, and your rights under the KVKK.

The personal data you share through the contact form on our website, such as your name, surname, phone number, and email address, will be processed for the purposes of carrying out necessary operations to benefit individuals from the products and services provided by our company, conducting relevant business processes, and being able to communicate with you regarding the subject you have applied for. Additionally, we have no absolute control over the information you share through this form. We advise you not to share any personal data or sensitive personal data that you do not wish us to know.

These personal data will be processed automatically based on the legal grounds specified in Article 5 of the KVKK, namely “it is mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the relevant person” and “it is necessary for the data controller to fulfill a legal obligation.” The data will only be shared with relevant judicial authorities in the event of legal disputes.

You can send your requests, which fall under Article 11 of the law regulating the rights of the relevant person, in writing to the address: Ortabayır Mahallesi Oto Caddesi No: 3 Kağıthane – Istanbul, or to the email address info@leventhastanesi.com.tr in accordance with the “Communiqué on the Procedures and Principles for Applying to the Data Controller.”
