Corporate General Clarification Text Regarding the Processing of Personal Data

As Levent Hospital A.Ş. (“Levent Hospital”), we have prepared this clarification text to inform you—our Visitors, Online Visitors, Customers, Potential Customers, Supplier Employees, and Supplier Representatives—regarding the processing, storage, and transfer of your personal data within the framework of the Personal Data Protection Law No. 6698 (“KVKK”) and related legislation and legal regulations.

  1. What Personal Data Categories Do We Process and For What Purposes?

Your personal information may be processed by Levent Hospital A.Ş. in accordance with the principles set forth in Article 4.2 of the KVKK:

(i) Compliance with the law and honesty,

(ii) Accuracy and, when necessary, currency,

(iii) Processing for specified, explicit, and legitimate purposes,

(iv) Being related, limited, and proportional to the purpose of processing,

(v) Retention for the period required by the relevant legislation or the purpose for which it was processed.

Based on our ongoing business relationships with our partners (a person may fall into multiple categories), the following purposes may apply:

A. Our Visitors

Physical security information of our visitors (e.g., camera recordings) will be processed for the purpose of ensuring physical security.

B. Our Online Visitors

Processing of information such as transaction security data (e.g., your IP address), marketing information (e.g., cookie records), and identity and contact information (e.g., your email address, if provided) will be conducted for:

a) Conducting activities in compliance with legislation,

b) Managing information security processes,

c) Following up on requests/complaints,

d) Conducting communication activities,

e) Providing information to authorized persons, institutions, and organizations,

f) Enabling access to our website via the internet.

C. Our Supplier Employees

Identity information, contact information, physical security information, and professional experience information (e.g., in-service training information) of our supplier employees will be processed for:

a) Conducting communication activities,

b) Managing/monitoring business activities,

c) Ensuring physical security,

d) Managing supply chain processes,

e) Conducting logistics activities,

f) Conducting activities in compliance with legislation,

g) Managing occupational health and safety activities.

D. Our Supplier Representatives

Identity information, contact information, physical security information, customer transaction information, financial information, marketing information, and legal transaction information of real person suppliers or representatives of legal person suppliers will be processed for:

a) Conducting/monitoring business activities,

b) Managing the procurement processes of goods/services,

c) Conducting financial and accounting operations,

d) Managing contract processes,

e) Conducting investment processes,

f) Ensuring physical security,

g) Following up and executing legal affairs.

  1. What Are the Methods of Collecting Your Personal Data?

The personal data specified in categories above are collected through physical means such as order forms, contracts, and visitor forms, or via information systems and electronic devices (e.g., telecommunication infrastructure, computers, and phones), third parties (e.g., KKB and Findeks), our website, and other documents declared by the relevant person, either automatically or manually.

  1. What Is the Legal Basis for Collecting Your Personal Data?

Your personal data is processed by Levent Hospital A.Ş. based on the legal grounds specified in Article 5 of the KVKK for achieving the purposes explained above:

(i) Explicitly stipulated by laws,

(ii) Necessary for establishing or fulfilling a contract, directly related to the contract parties,

(iii) Mandatory for the data controller to fulfill a legal obligation,

(iv) Mandatory for the establishment, exercise, or protection of a right,

(v) Mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the relevant person.

If the data processing activity cannot be based on one of the above legal grounds, your personal data will be processed based on your explicit consent.

  1. Do We Transfer Your Personal Data to Third Parties?

a) Personal data of our visitors may be shared with law enforcement and judicial authorities for the resolution of legal disputes and when required by the relevant legislation.

b) Personal data of our online visitors may be shared with judicial authorities and competent public institutions for the resolution of legal disputes and when required by the relevant legislation.

c) Personal data of our customers may be shared with authorized public institutions and organizations within the scope required by legal regulations, to ensure compliance with legislation, to follow up and execute legal affairs, and with community companies, suppliers, cargo companies, insurance companies, banks, and financial advisors as necessary to conduct business operations/monitoring and risk management processes. Additionally, such personal data may be shared with our legal office and judicial authorities for potential future legal disputes as evidence.

d) Personal data of our potential customers may be shared with our legal office and judicial authorities for potential future legal disputes as evidence.

e) Personal data of our supplier employees may be shared with authorized public institutions and organizations as required by legal regulations, to ensure compliance with legislation, to follow up and execute legal affairs, and to provide information to authorized persons, institutions, and organizations. Such data may also be shared with community companies, suppliers, cargo companies, banks, and financial advisors as necessary to fulfill the service adequately and may be used as evidence in future legal disputes.

f) Personal data of our supplier representatives may be shared with authorized public institutions and organizations as required by legal regulations, to ensure compliance with legislation, to follow up and execute legal affairs, and to provide information to authorized persons, institutions, and organizations. Such data may also be shared with community companies, suppliers, cargo companies, banks, and financial advisors as necessary to fulfill the service adequately and may be used as evidence in future legal disputes.

  1. How Can You Exercise Your Rights Related to Your Personal Data?

You can submit your requests, which fall under Article 11 of the KVKK regarding the rights of individuals, through the “Data Controller Application Form” prepared to facilitate you on our website at “www.leventhastanesi.com.tr” in accordance with the “Communiqué on the Procedures and Principles for Applying to the Data Controller.”
